it says to make Death scythe like this

Smithing (93) Alchemy (59) Woodworking (?)
Crystal: Fire
Ingredients: Steel Ingot x3, Darksteel Scythe, Yew Lumber, Grass Cloth, Fiend Blood
Result: Death Scythe
HQ #1: Death Scythe +1

i put those ingredients in and i get a nice message saying
Synthesis canceled. That combination of materials cannot be
synthesised.

im 86+2 smithing and 58 alchemy. i saved 21 darksteel scythes from skill up to make these for skill up, im pissed.

It's too bad that the recipe is not correct as listed; that's one of those recipes that showed up on some sites but not others, and it's hard to get verification either way until someone actually tries it.

Can you check with the NPC Craftsmen to see if they give a similar recipe? The recipe as listed may just be off by one or two items. If there's no recipe like that from the guild, I'll remove the recipe entirely; if you (or anyone else, for that matter) can find the accurate info for a recipe that uses the Darksteel Scythe in it, that would certainly be a help.

That recipe with the Darksteel Ingots is certainly correct, and has always been listed here as a recipe for the Death Scythe. The big question is whether there is also another recipe that uses the Darksteel Scythe in it; the one listed is apparently incorrect, but it may just be missing some information.

After checking further, I've removed that recipe entirely; if anyone gets accurate info on a second recipe for the Death Scythe, it can be put in. On a related note, there's desynth info for both the Death Scythe and Darksteel Scythe now.
